All Grades School Photo Package Preview - All students will have their photo taken for class pictures -
All Grades Health Screening & Prevention information from Calumet County - No screening will be completed this year due to the pandemic
All Grades Parent Volunteer Policy and Documentation Form - 10/20 hours required volunteer service per family unit

Cell Phone Permission Form - for any students who brings a cell phone to school

Grade 1-8 Fluoride treatments are unavailable for 2020-21.
HOT LUNCH PROGRAM
All Students have their own individual hot lunch account. Make checks payable to SJSH Hot Lunch
Cost: Student Lunch (includes one milk): $3.00 Milk Only: .25 cents

Grade 3K & 4K Milk Program Costs - Milk is available to students during morning snack. If your child does not drink milk water will be available.

STUDENT IMMUNIZATIONS
Immunization Forms Immunization Law Requirements
New students - complete immunization form on or before Orientation Day
Kindergarten - required kindergarten immunizations
Grade 6 - required Tdap or Td check with your doctor
Waiver - required for medical, religious & personal reasons

MEDICATIONS
Prescription medications and Non-prescription medications/supplements must e brought to the office in their original container. This Includes cough drops.

Medication Consent Form - Keep for future use unless your child needs medication/supplements at the beginning of the school year. This form MUST be signed by your doctor if the medication/supplement is a prescription.
